Immunization Records in Central Oregon

It’s important to keep track of your and your child’s immunization records. You should bring these personal immunization records to all medical appointments. That way, you avoid repeating vaccinations that you or your child already had.

Document any information in writing, including the vaccine name (e.g., "MMR"), date given, and provider or clinic name. Most state health departments can provide you with a child immunization record card. If you do not have a child immunization record card, download one for free. (You need Adobe Reader to view this file. If you don’t have it, download a free copy.)

If you can’t find your child’s immunization records, call your child’s school. The school often keeps these records. You can also try to contact:

Oregon Immunization ALERT 
This organization gives authorized parents or guardians a copy of their child’s immunization record in Oregon. ALERT keeps immunization information on all ages of Oregonians but concentrates on children age birth through 17.
You Need to Know: Before releasing immunization records to a parent or guardian, ALERT must receive a signed agreement that the person requesting the immunization record is the legal parent or guardian of the child. ALERT will also forward a copy of the immunization record to the child’s health care provider, school, or child care facility, if requested.

Getting Immunization Records from Other States

Unfortunately, there is no national organization that keeps this information. Some states have immunization registries that keep electronic records of all vaccines given. To ask about the possibility of such a registry in your area, call your state immunization coordinator or the coordinator of any states where you used to live.
